[Resolvido] Inclui mas nao volta para a index

Pessoal ja estou loko aqui pra fazer isso funcionar

esse codigo ele inclui tudo certinho, porem quando

clico no OK ele inclui no banco mas continua na mesma

tela com todos os dados preenchidos.

Eu queria que ele incluise e voltase para a index.php

onde posso enta errando?
